Subsequent treatment with Zithromax improved these men and women dramatically, showing that the test was right.”

Chronic Lyme Disease and Co-infections: Differential Diagnosis.

In Lyme disease concurrent infections frequently occur. The clinical and pathological impact of co-infections was first recognized in the 1990th, i.e. approximately ten years after the discovery of Lyme disease. Their pathological synergism can exacerbate Lyme disease or induce similar disease manifestations. Co-infecting agents can be transmitted together with Borrelia burgdorferi by tick bite resulting in multiple infections but a fraction of co-infections occur independently of tick bite. Clinically relevant co-infections are caused by Bartonella species, Yersinia enterocolitica, Chlamydophila pneumoniae, Chlamydia trachomatis, and Mycoplasma pneumoniae. In contrast to the USA, human granulocytic anaplasmosis (HGA) and babesiosis are not of major importance in Europe. Infections caused by these pathogens in patients not infected by Borrelia burgdorferi can result in clinical symptoms similar to those occurring in Lyme disease. This applies particularly to infections caused by Bartonella henselae, Yersinia enterocolitica, and Mycoplasma pneumoniae. Chlamydia trachomatis primarily causes polyarthritis. Chlamydophila pneumoniae not only causes arthritis but also affects the nervous system and the heart, which renders the differential diagnosis difficult. The diagnosis is even more complex when co-infections occur in association with Lyme disease. Treatment recommendations are based on individual expert opinions. In antibiotic therapy, the use of third generation cephalosporins should only be considered in cases of Lyme disease. The same applies to carbapenems, which however are used occasionally in infections caused by Yersinia enterocolitica. For the remaining infections predominantly tetracyclines and macrolides are used. Quinolones are for alternative treatment, particularly gemifloxacin. For Bartonella henselae, Chlamydia trachomatis, and Chlamydophila pneumoniae the combination with rifampicin is recommended. Erythromycin is the drug of choice for Campylobacter jejuni.

Er zijn ongeveer 100 herpesvirussen bekend, waarvan er 8 actief kunnen zijn bij de mens. Enkele onderzoekers van de SUNY Upstate Medical University in Amerika hebben onderzoek gedaan naar polyneuropathie. Zo stellen zij dat polyneuropathie vaak wordt veroorzaakt door de ziekte van Lyme. Zij bespreken in hun onderzoek de medische situatie van een man met craniale zenuwstoornissen (die dysfagie, dysfonie en zwakte in het gezicht veroorzaken). Varicella Zoster werd in zijn hersenvocht gevonden. Varicella Zoster staat in de volksmond bekend als waterpokken en gordelroos.
Bij de klinische beoordeling wordt dit fenomeen zelden gemeld, maar wel waargenomen met een aantal herpesfamilie-virussen. Zij stellen dan ook dat op de afdeling spoedeisende hulp een klinisch vermoeden moet worden opgeworpen voor een VZV-infectie, zelfs in de afwezigheid van huiduitslag bij patiënten met meerdere craniale zenuwparasieën.

Herpesvirussen (Herpesviridae) zijn een groep virussen die bij mens en dier ziekten veroorzaakt, waarvan bij de mens waterpokken en gordelroos (varicella-zostervirus), de koortslip, herpesstomatitis, herpesfijt van de vinger en herpes genitalis (herpes simplexvirus) de bekendste zijn.
Andere zijn het cytomegalovirus en het Epstein-Barrvirus, dat de ziekte van Pfeiffer veroorzaakt. De virussen blijven levenslang, veelal latent, aanwezig in de geïnfecteerde gastheer. Het latent aanwezige virus vermenigvuldigt zich niet en blijft onopgemerkt door het immuunsysteem. Zo nu en dan kan het virus opvlammen.[1]

Oorzaken. Eén van de belangrijkste oorzaken van perifere neuropathie is diabetes mellitus. Schade aan zenuwcellen wordt daarbij veroorzaakt door een hoge bloedglucoseconcentratie. Mogelijk leidt dit tot micro-angiopathie en een verminderde bloedtoevoer van de zenuwen. Een andere hypothese is dat een hoge glucoseconcentratie in de zenuwcel kan leiden tot schade aan het axon (ofwel zenuwceluitloper) en demyelinisatie.1-3 De pijnklachten van diabetische neuropathie beginnen gewoonlijk in de voeten en kunnen later ontstaan in de handen (Gebu 2007; 41: 83-92).

Een perifere neuropathie kan ook het gevolg zijn van een infectie, zoals herpes zoster (postherpetische neuralgie), de ziekte van Lyme (neuroborreliose) of een infectie met het humane immunodeficiëntievirus (HIV).

De neuropathie kan ook een metabole oorzaak hebben, zoals een vitaminedeficiëntie, bijvoorbeeld een tekort aan thiamine (vitamine B1), pyridoxine (vitamine B6) of cyanocobalamine (vitamine B12).

Verder komen perifere neuropathieën ook voor bij immunologische stoornissen (bv. het syndroom van Guillain-Barré), bij verschillende soorten tumoren (paraneoplastische neuropathie bij lymfomen en myelomen), kunnen ze het gevolg zijn van een erfelijke aandoening, of kunnen ze ontstaan als zenuwen worden afgeklemd (drukneuropathie), na een trauma, na langdurig alcoholgebruik of na intoxicaties met chemicaliën en zware metalen. Ook geneesmiddelen kunnen perifere neuropathieën veroorzaken.1 2

The Centers for Disease Control and Prevention (CDC) holds as its mission the protection of U.S. citizens from disease regardless of whether the disease threat is “chronic or acute, curable or preventable, human error or deliberate attack.” The CDC includes in its “Pledge to the American People” that all public health decisions will be based on the highest quality scientific data that is derived openly and objectively; to place the benefits to society above the benefits to the institution; and to treat all persons with dignity, honesty, and respect (1).

However, in the case of Lyme disease, the CDC is not fulfilling its mission or its pledge to protect the American public from a disease this governmental agency readily acknowledges it has been underestimating for years (2).

Based on epidemiological reports of notifiable diseases maintained by the CDC, Lyme disease is overall one of the top three notifiable infectious diseases in the United States. Although commonly referred to as the leading vector-borne disease in the United States, a review of CDC surveillance data on all infectious diseases on the “Nationally Notifiable” list of diseases, shows that Lyme disease is second only to Chlamydia (a sexually transmitted disease) in terms of overall number of new cases annually.
